Join Our Patient Forum
We would like to know how we can improve our service to you and how you perceive our surgery and staff.
To help us with this, we have set up a patient forum so that you can have your say. We will ask the members of this forum to meet up to 6 times a year so we can feedback to you and so you can be involved in the practice.
We need young people, workers, retirees, people with long term conditions and people from non-British ethnic groups so we can ensure that we gain the views of all ages and patients with different backgrounds.
If you would like to join the forum, or just be a member who receives the minutes of the meetings and contacts us occasionally, please complete the form below and hand in at reception.

This is a group of patients who meet with the practice staff up to 4 times a year. The practice value this input from patients on how to develop our services. If you wish to join the patient forum, please ask at reception.
